A familiar reminder can help someone maintain an established medication routine. We discuss what support is appropriate and record the agreed approach in the care plan.
Supporting an established routine
Caregivers can offer reminders at the times identified in the plan and report concerns such as missed doses, difficulty following the routine or changes noticed during visits.
Clear boundaries and communication
Medication reminders are different from nursing medication administration. We assess whether reminders are sufficient or whether a nurse needs to be involved. Any assistance with packaging or other medication tasks must be specifically assessed and authorized in the plan.
Planning your care
Medication choices, doses and changes remain with the appropriate prescribing professional. Ask us about nursing support if administration is required.
