We discuss health and daily support needs, routines, preferences, mobility, emotional well-being and the people involved in your care.
A plan built together
The assessment informs a personalized care plan, including the type of support, proposed schedule and any further information or professional coordination needed.
Clarity before care begins
We explain the proposed services and pricing before you agree to care. Family members or other supporters can be involved with the client’s consent.
Planning your care
The initial assessment is complimentary. Ongoing services are discussed and agreed separately.
